Dr. Kenneth Dardick is a founding member of Mansfield Family Practice. He has lived in Mansfield since 1976. In the early 1980's his interest in child safety led him to form the Connecticut Safety Belt Coalition. As President of the CSBC he was instrumental in gaining passage of Connecticut's Child Safety Restraint Law, one of the first in the country. He has been active in health and public health activities both locally and state-wide, having served as a Councilor of the Connecticut State Medical Society, Chief of the Medical Staff at Windham Community Memorial Hospital, Chair of the Hypertension Task Force of the Connecticut chapter of the American Health Association, Director of Health for Ashford, Mansfield and Willimantic and Medical Advisor to the Eastern Highlands Health District. He holds a faculty position as Assistant Clinical Professor of Medicine at the University of Connecticut and is a member of the Faculty of Travel Medicine of the Royal College of Physicians and Surgeons of Glasgow (Scotland). His research interests include Lyme disease and tick-borne infections, and traveler's health and tropical diseases. Dr. Dardick is a graduate of Harvard Medical School and has studied at the London School of Hygiene and Tropical Medicine. He has held leadership positions in the American Society of Tropical Medicine and Hygiene (Councilor, American Committee on Clinical Tropical Medicine and Traveler's Health) and the International Society of Travel Medicine (Chair of the Examination and Certification Committee).
Nelson C. Walker II, MD
Dr. Walker joined Mansfield Family Practice in 1980. He had previously lived in the Mansfield area in the 1970's prior to attending the University of Connecticut School of Medicine. In addition to being dedicated to patient care at MFP, he has been very involved with the Connecticut Academy of Family Physicians, serving as member of the Board and President for a number of years. He was the editor of The Connecticut Family Physician for over 10 years and was on the Executive Committee for the Core Content Review of Family Physicians. He has been a member of the Connecticut State Medical Society Committee on Medical Aspects of Sports, and has written articles for their publication Sports Medicine. For 15 years Dr. Walker was Chief of the section of Family Practice at Windham Community Memorial Hospital and has also served on numerous committees. He helped establish the local Hospice program in the 1980's and served on the advisory board until 2003. As Assistant Clinical Professor in the Department of Family Medicine, University of Connecticut School of Medicine, Dr. Walker provides clinical training of 3rd year medical students. With his wife Joanne he raised three sons and was very involved with youth soccer in Mansfield for many years. He enjoys outdoor activities including canoeing, sailing, skiing and snowshoeing.
Ayaz T. Madraswalla, MD
Dr. Madraswalla joined Mansfield Family Practice in 1995. He received his undergraduate degree from Boston University in 1986. He then studied medicine at Albany Medical College and graduated in 1990. He then went on to complete his Residency in Family Practice at the University of Colorado in Denver in 1993. He was in practice in Virginia prior to becoming a part of our practice. Dr. Madraswalla takes an active role in his profession by serving on the Board of Directors for the Connecticut Academy of Family Physicians since 1999, as well as past Councilor to the Connecticut State Medical Society. He has served as President of both the Connecticut Academy of Family Physicians and the Windham County Medical Association. He also enjoys teaching medical students and holds a position as an Assistant Clinical Professor at the University of Connecticut Medical School. Dr. Madraswalla serves as School Physician for Windham Regional Vocational Technical School. His interest within medicine include pediatrics, preventive health care, allergy and asthma treatment along with smoking cessation. Dr. Madraswalla is married with two children and enjoys many activities including reading, cooking, tennis, skiing, traveling, bicycling and spending time with his family.
Anna M. Jamrozik, MD
Dr. Jamrozik became part of Mansfield Familiy Practice in 1999. She received her MD from Jagiellonian University, Collegium Medicum in Krakow, Poland in 1986. She completed her internship and residency in Pediatrics at Collegium Medicum from 1986-1990. She completed a second residency in Otolaryngology-ENT at Collegium Medicum from 1991-1994 and a third residency in Family Practice at St. Joseph's Medical Center in Reading, Pennsylvania from 1996-1999. Dr. Jamrozik is married and has two children.
Ross L. Winakor, MD, MPH
Dr. Winakor joined Mansfield Family Practice in 2003. He is a graduate of Tulane University School of Medicine in New Orleans, Louisiana. He received a Masters degree in Public Health (MPH) in Epidemiology from Tulane University School of Public Health. Dr. Winakor completed his Residency in Family Practice at Hunterdon Medical Center in Flemington, New Jersey, where he was elected to serve as Chief Resident. He completed a Fellowship in Primary Care Sports Medicine at the University of Pittsburgh Medical Center, where he served as an Assistant Team Physician for the University of Pittsburgh. While a resident, Dr. Winakor has served on the Board of Trustees of the New Jersey Academy of Family Physicians and was also national commttee representative of the American Academy of Family Physicians. Since 2004 he has also served on the Board of Directors of the Connecticut Academy of Family Physicians and he was also elected to serve on the Connecticut State Medical Society Committee on the Medical Aspect of Sports. Dr. Winakor joined Mansfield Family Practice in 2003.
Maura E. Baker, MSN APRN
Maura E Baker is a Board-certified Family Nurse Practitioner who joined Mansfield Family Practice in 1983. She is a graduate of Dartmouth College and Duke University and has served on the Board of Directors of the CT Nurse Practitioner Group for several years. She has extensive experience in adventure travel, and her special interests include travel/wilderness medicine, women's health care, pediatrics and promotion of healthy lifestyles.
Kimberly Beaulieu, PA-C
A New Hampshire native, Kimberly Beaulieu joined Mansfield Family Practice in 1998. She completed her undergraduate education at the University of New Hampshire in 1988 and continued her studies at the Yale School of Medicine, graduating from the Physician Associate Program in 1992. Ms. Beaulieu has been a member of the American Academy of Physician Assistants since 1990. She is married with three children. Tammy Gendron, MHS, PA-C
Tammy joined Mansfield Family Practice in 2008 as a physician assistant. A native of eastern Connecticut, Tammy decided to return home to practice medicine. She completed her undergraduate degrees at Eastern Connecticut State University receiving a BS and BA in 2000 and a BGS in 2004. Tammy completed her master’s degree with her physician assistant certificate in 2007 at Lock Haven University in Lock Haven, PA. She has been an active member of the American Academy of Physician Assistants since 2005.
